Eligibility

Are you eligible for this plan? As an enrollee you must work or reside within the Scott & White Health Plan service area to be eligible for this plan. (See map PDF document)

We are pleased you have selected Scott & White Health Plan for your health care needs. Effective September 1, 2011 the following changes will be made to your Scott & White Health Plan policy through TRS-ActiveCare.

Summary of improvements and changes:

  • Our plan now has a $750 deductible.
  • The annual out-of-pocket maximum has changed to $3,000. This means that the most you would pay in a year is $3,000 for individual coverage.
  • The family out-of-pocket maximum remains unchanged at $6,000.
  • The primary care office visit copay remains at $20 per visit.
  • The specialty office visit co-pay is now $50 per visit.
  • For allergy serum, you pay 20% after deductible.
  • Outpatient surgery: $100 co-pay plus 20% after deductible.
  • Generic prescriptions are now only $3!

What hasn't changed is our commitment to providing you and your dependents with personalized high-quality health care.
