Requesting An Authorization From
The Health Services Division

Plan-approved physicians and/or providers may request prior authorization of specific services through submission of the SWHP Authorization Forms, to include any additional pertinent information as attachments as needed for the determination to be rendered. The form must be completed accurately and with all required fields addressed. (Operational definitions of the fields are included with the form.)

Once completed, the form must be printed and have an original signature (no stamp signatures allowed) of the requesting physician and/or provider. The form may then be faxed to the SWHP Health Services Division in Temple, Texas through a toll free fax server for processing.

Determinations will be made as quickly as possible when all relevant information is provided according to the Texas Department of Insurance (TDI) regulatory standards. If an incomplete form is received, it will be returned and not processed until such time as the required information is submitted.

Questions /clarifications about authorization requests that have been submitted may be called to the Health Services Division at toll free # 1-888-316-7947 (local #254-298-3088) during normal business hours. After hours and urgent/emergent calls can be made to these same numbers (which will be answered by the S&W Hospital Operators). Please ask for a SWHP Continuing Care Coordinator to be paged.

Once determinations have been made, the Health Services Division will notify the requesting physician and/or provider(s) verbally and in writing of the determination. Please note that the Health Services Division will make up to three attempts to provide verbal answers by phone. If no verbal contact is accomplished, the notice will occur only through the written correspondence to the Member that is also being provided to the requesting physician and/or provider(s).
