Health Plus Saver 3000 PDF document and Health Plus Saver 5000 PDF document

We off two Saver plans for 2009. These Health Savings Account (HSA) plans are great for entrepreneurs, early retirees and anyone looking for the most affordable coverage with a low premium and higher deductible.

  • Monthly premiums based on age and gender
  • Once you reach your deductible the plan pays 100% on many items
  • Lifetime maximum $5,000,000 per member
  • Preventive Services are covered at 100% before deductible
  • Includes $3,000 prescription coverage after deductible
  • Can protect you against high or unexpected medical costs
  • Offers flexibility, control and portability
  • Can offer significant tax advantages

Health Plus Portfolio

We offer four Portfolio plans for 2009. This plan gives the member access to doctor visits and medication without paying a deductible and protection when something major happens.

  • Monthly premiums based on age and gender
  • Offers choices on yearly deductibles - $750 PDF document, $1,500 PDF document, $2500 PDF document, or $5000 PDF document
  • Has standard benefits with many of the deductibles waived
  • Lifetime maximum $5,000,000 per member
  • $30 copayments for unlimited office visits
  • $0 copayment on preventive care
  • Preventive services covered at 100% before deductible
  • Includes $1,000 prescription coverage
  • Optional dental and term life plans1

Health Plus VI PDF document

This plan gives the member basic medical coverage that will allow them to see a doctor without having a high out-of-pocket expense.


  • Monthly premiums based on age and gender
  • $250 deductible for certain services
  • Lifetime maximum $5,000,000 per member
  • $30 office visit copay
  • $200 in-patient copayment; $1,000 maximum per hospital stay
  • Optional prescription drug coverage
  • Optional dental and term life plans1

Health Plus V PDF document

This plan gives the member rich benefits with less risk - which means higher monthly premiums but less out-of-pocket expenses when something major happens.


  • Monthly premiums based on age and gender
  • No deductible
  • No lifetime benefit maximum
  • $25 copayments for unlimited office visit
  • $100 in-patient copayments; $500 maximum
  • Limits the member's out-of-pocket expense per hospital stay
  • Optional prescription drug coverage
  • Optional dental and term life plans1
