Life Benefits
If you (or anyone insured under this plan) die while covered for life benefits, the designated beneficiary will be paid the amount of life benefits that is in effect on the date of death.
Insurance Benefits to Help During a Terminal Illness
Up to 50% of life benefits may be paid if an insured member is terminally ill with a life expectancy of 24 months or less, The amount will not be greater than $12,500 or less than $5,000. Insured members should consult their tax or legal advisors to determine the tax consequences of accelerated benefit payments.
Protection While You are Disabled
Insured members who become disabled prior to age 60 and remain so for nine consecutive months, are eligible to have premiums waived while totally disabled. Life insurance premiums will be waived as long as you are disabled and on the Scott & White Health Plan. This coverage will continue until you reach age 65 or discontinue the Scott & White Health Plan. Initial evidence establishing your total disability will be required and may be required periodically thereafter.
Ability to Continue Coverage
If an insured member or dependent under the Health Plus Plans has his or her life insurance benefits terminated, then the insured may be eligible to apply for an individual life insurance policy without the need to provide evidence of insurability.

Accidental Death and Dismemberment Benefits
If you (or anyone insured under this plan) die as a result of a covered accident, an additional benefit will be paid equal to the life insurance benefit. The amount that may be paid for losses other than death are described in the Table of Covered Losses and Benefit Amounts below. This Accidental Death and Dismemberment benefit is paid if the loss occurs within 90 days of the accident. No more than the full amount of the Accidental Death and Dismemberment benefit will be paid for a covered loss that results from injuries sustained in one accident. Accordingly, the maximum payout under the policy will be two times the amount of the basic life insurance coverage (life benefit plus the full amount of the Accidental Death and Dismemberment benefit) plus the Seat Belt and Air Bag benefits, if applicable.

Additional Benefits for Seat Belts and Air Bags
The Seat Belt benefit provides an additional 10% (minimum of $1,000 and a maximum of $10,000) and the Air Bag benefit provides an additional 5% (minimum of $100 and a maximum of $10,000) of the full amount of the accidental death and dismemberment benefit. These benefits are payable if you die as a result of injuries sustained in an accident while driving or riding in a registered vehicle equipped with an Air Bag(s) and/or wearing a properly fastened seat belt.
Common Carrier Benefit
If an insured member's loss of life results from riding a a fare-paying passenger in any public common carrier, such as a bus, train or airplane, the company will pay a total benefit payout, including life insurance, of three times the life benefit amount, which is composed of the Life benefits, plus the full amount of the Accidental Death and Dismemberment benefit, plus the full amount of the Common Carrier benefit.
Accidental Death and Dismemberment Exclusions
Benefits will not be paid for any loss caused or contributed to by: physical or mental illness or infirmity, or the diagnosis or treatment of such illness or infirmity; infection, other than infection occurring in an external accidental wound; suicide or attempted suicide; intentionally self-inflicted injury; committing or attempting to commit a felony; the voluntary intake or use by any means of: any drug, medication, or sedative, unless it is: taken or used as prescribed by a Physician, or an "over the counter" drug, medication or sedative taken as directed, or alcohol in combination with any drug, medication, or sedative; or poison, gas, or fumes; or war, whether declared or undeclared, while in military service; or act of war, or participation in an insurrection, rebellion, riot, or terrorist act. Benefits will not be paid for any loss if the injured party is intoxicated at the time of the incident and is the operator of a vehicle or other device involved in the incident.
Contract Terms & Conditions
This benefits description is based on the contractual provisions contained in the Master Group Policy Form GPNP99-DSC-SWM-LIFE issued in Texas to Scott & White Health Plan. Coverage will be provided through member participation in the Master Group Policy, with certificates of insurance (Form GCERT2000 DSC-SWM-LIFE-FAM) issued to each insured active member. For members residing in any state other than Texas that validly exercises extraterritorial jurisdiction with respect to the coverage provided under the Scott & White Health Plus Plan, the plan will be modified to meet all applicable laws.
Effective Date - IMPORTANT:
Coverage becomes effective on the date set by the Scott & White Health Plan after the initial premium has been paid, subject to evidence of good health as necessary.
