Health Plus Individual Plans Eligibility
To be eligible you must:
- Work or reside in the Scott & White Health Plan service area.

Be between 18 and 64 years old - To subscribe as a policyholder you must be 18 years of age. Dependents can be covered to age 25 as long as their permanent legal residence is the same as the Subscriber's. Permanent Legal Residence is defined as the address as which the dependent intends to live for the contract year. For students, it will be presumed that their Permanent Legal Residence will be that of the parent with whom they resided at the time of enrollment

Prove evidence of good health - To provide evidence of good health, all applicants must submit a completed medical history form, included in the individual enrollment application. In absence of complete medical records, a physical may be required at the applicant's expense.
The following are examples of medical conditions that would automatically disqualify an applicant: tobacco usage, current pregnancy, exceeds height/weight guidelines, heart disease, cancer, stroke, diabetes, kidney disease, AIDS as well as any other conditions that would be expected to require above average utilization of medical services.
- Pay Appropriate Fees - If you are accepted, we will notify you of the effective date of your coverage. You must pay the first month's premium prior to the coverage effective date.
