Adding Dependents



Newborns - Newborns are covered for the first 31 days. The insured has 60 days from date of birth to notify SWHP in writing by submitting the Application/Enrollment form. Coverage may be terminated on the 32nd day if the proper notification has not been received.

Adoption - At the option of the insured, adopted children are covered either on the date of adoption or on the date the lawsuit seeking adoption is filed. The insured has 60 days from whichever of these 2 dates is elected to notify SWHP in writing by submitting the Application/Enrollment form.

Court Order - If a court has ordered coverage to be provided for a child under a covered employee’s plan and the employee requests enrollment of the child no later than the 31st day after the date on which the court order is issued, then the child may be enrolled and coverage will be effective on the day of notification to the employer. Notification must be received in writing by submitting an Application/Enrollment form and must include the following documentation:

  • The coverage page of the court order which includes the date of the order and the parties involved;
  • The pages from the court order which describe the specifics of the medical support requirement; and
  • The final page which includes the judge’s signature and seal.

Grandchildren - Notification must be made to SWHP in writing by submitting the Application/Enrollment form within 31 days of the dependent satisfying the eligibility requirements for coverage to be effective. Coverage will be effective date of birth.

Marriage - Notification must be made to SWHP in writing by submitting the Application/Enrollment form within 31 days of the date of marriage for coverage to be effective. Coverage will be effective on the first day of the next month following receipt of the signed Application/Enrollment form by SWHP.

Informal Marriages - Informal or Common law marriages require a Certificate of Informal Marriage from the courthouse. The date of certificate serves as the date of marriage. Notification must be made to SWHP in writing by submitting the Application/Enrollment form within 31 days of the date of marriage. Coverage will be effective on the first day of the next month following receipt of the signed Application/Enrollment form by SWHP.
