Enrollment

If you are an active employee or retiree, you can enroll online from July 28 through August 22 at the ERS Web site, www.ers.state.tx.us. You may also enroll traditionally using an enrollment form available from your Benefits Coordinator or online at the ERS Web site, Forms. This form should be returned to your Benefits Coordinator. If you enroll online, you may select your Primary Care Physician (PCP) and the PCPs for your dependents by using the reference number for your doctor listed in the Scott & White Health Plan online provider directory. You may also select your PCP on a Supplemental Information Form, which should be sent to the selected carrier.
