As a Health Plus member, you are also eligible for $25,000 in Life Insurance coverage, underwritten by MetLife. Additionally, you may be eligible for $25,000 of coverage for your spouse and $10,000 of coverage for dependent children.1
If you would like to apply for life insurance, please fill out the life insurance application and return to Scott & White Health Plan. We will submit your request to MetLife for review. As soon as we receive approval from MetLife we will let you know if your application has been accepted.
