Welcome to the renewal center!
This center contains all the forms you need to change your current coverage. 

You recently received your renewal letter in the mail giving you our good news about your November 1 renewal!  The cost of your medical plan will not change when it renews on November 1, and your benefits will remain the same. If you also have prescription drug coverage, there will be no change in cost or benefits.

Please note:  Even though our rates remain the same, some of you have had birthdays and will move to a new age bracket with higher rates.   If you or another adult on your plan will be age 30, 35, 40, 45, 50, 55, 60, or 65 on or before October 31, 2011, we want to help you review your choices for a lower-cost plan.    Please call our renewal team at 877-845-3902 Monday - Friday, 8 to 5.

Please note that this is the only time during the year (renewal) that you can add or drop optional coverages (prescription drug, dental, and life insurance).  Applications for prescription drug coverage are subject to medical review---prescription drug coverage is not guaranteed.  

Dental rates will change on November 1.  If you currently have dental coverage, provided by MetLife, please note the following change: 

Some helpful things for you to remember...

1. Your current plan will automatically renew on November 1.
2. All deductibles and benefits start over on November 1.
3. As always throughout the year, Doctor visits may result in more than a copay if additional medical services are billed at the time of your visit. Your deductible may apply for these additional services, or your plan may require you to pay 20% or 30% once your deductible has been met.

If you want to make changes you will need to submit the appropriate form from the list below. If you need us to send you a form, call us at 877-845-3902. Completed forms must be received in the Scott & White Health Plan office by November 1, 2011.

*Changing to a new medical or prescription benefit plan may require Medical Underwriting. Acceptance is not guaranteed. If Medical Underwriting denies your plan change, you may remain on your current medical plan.
