The Dialog Center - Disease/Condition Care Guidance Programs and Shared Decision Making*
Our disease and condition care guidance programs provide you with the tools and resources to learn more about your health and to continue to make positive changes in your life. These programs addressing 65 different diseases and conditions are available to Scott & White Health Plan commercial members* by calling a Health Coach at 877-505-7947.
Additional information can be accessed through the Dialog Center.
Our new Dialog Center allows you to contact a Health Coach by e-mail. You can also
- look up information on medical conditions and drugs,
- watch videos made by patients and/or doctors,
- use interactive tools such as quizzes and calculators
- check on symptoms you may be having, or
- just read the latest health news.
The Dialog Center also contains our Shared Decision Making resources. Our resources give you reliable tools and information to help you make important medical decisions. Use them to become better informed about your condition, so you can work more closely with your doctor when discussing your treatment.
* Please note: Currently, our SeniorCare (Cost) plan members are not eligible to receive calls from Health Coaches, but they may use the toll-free number and Dialog Center website for health information and tools.
VitalCare Nurse Advice Line
Nurses are available 24 hours per day, 365 days per year. Our nurses can give you information about how to take care of yourself at home or can help you decide if an appointment, an urgent care visit or an emergency room visit is best for your symptoms. If you want to talk to a nurse, call 877-505-7947. The nurse advice line is available to all SWHP members.

Our programs offer tools and resources that help you make healthy lifestyle choices to help prevent disease and manage medical conditions.
Basic Wellness and Disease Management Programs
All Scott & White Health Plan members automatically have access to our basic wellness and disease management programs and incentives at no cost.
Educational Materials and Tools
We provide education about asthma, cardiovascular disease, COPD, diabetes, hypertension, behavioral health, and maintenance medications.
Customized Wellness & Incentive Programs
Employers with 100 or more employees can choose from a menu of customized wellness programs, onsite health screenings and tailored, meaningful incentives.
VitalCare Health Education Audio Reference (HEAR) Library
For those who want to listen to information about a health topic, Scott & White Health Plan offers the Health Education Audio Reference (HEAR) Library, with over 300 English-language and 25 Spanish-language topics.
