Scott and White Health Plan does many things and offers many programs to keep its members healthy. It also has programs for members who have multiple chronic conditions or have complicated or very serious medical issues facing them. The program which is designed to help our members with these is called case management. Your doctor can refer you to this program or you can refer yourself by calling 254-298-3088 or 888-316-7947 or by requesting a screening online. If you or a member of your family has a qualifying condition, you may receive a phone call directly from our nurse case managers. They are trained to help you navigate the health care system and assist you or your family member in managing your medical issues.
