Complex Care Guidance

Scott & White Health Plan is pleased to announce its Complex Care Guidance Program. This program is available to you with your complex care needs due to serious illnesses or injuries. This will help you get the greatest value from your health benefit plan.

Complex Care Guidance is an enhancement to our care guidance programs. There is no additional cost to you.

How does the program work?

Our personal nurse case manager will be available for the following:

• Call you at your convenience to provide assistance and support.
• Answer your questions and provide education for you and your family, helping you to understand your condition and the physician's plan of care.
• Work with you, your family, and physician to help with arrangements for your care at home.
• Help you to set goals and create your personal plan to improve your health.
