Breast Cancer Awareness Drawing

If you are a Scott and White Health Plan (SWHP) Commercial member and get a mammogram each year, we have news for you! SWHP is promoting Breast Cancer Awareness by holding a drawing for a $250 gift card, just for getting your mammogram. There will be four $250 gift cards given away.

To be eligible for the drawing:

• Mammogram must be done between 11/1/2010 and 9/30/2011.
• You must send a copy of your mammogram results letter to the address below.
• You must write your Member Number (found on your Medical Plan Card) on the letter before mailing back.
• The envelope must be postmarked by 10/10/2011.

Drawing Date: The drawing will be held 10/15/2011.
