Scott & White Health Plan

Please follow these steps to become a SWHP provider:

1. Download the Provider Data Sheet and the SWHP Guidelines for Selection and Continued Affiliation of Practitioners forms located on our website under the Providers section. Please complete these two forms and fax them with a letter of interest and curriculum vitae to Scott & White Health Plan Provider Relations at 254-298-3044. Please include any areas of specialty, your locations and any other pertinent information in the letter of interest.
2. Scott & White Health Plan will make a determination regarding your participation in the network. You will either receive a contract, or a declination letter.
3. If the contract terms are acceptable, please sign and send back along with a copy of your Texas standardized credentialing application. We outsource primary source verification, and the company we use (Med-Advantage) may contact you for information. Please respond timely or it will delay your effective date.
4. It usually takes 60 to 90 days to credential a new provider. All new providers must be approved by the Scott & White Health Plan Credentialing Committee. The Credentialing Committee meets once a month. If you are approved, you will be effective the first of the month following your approval by the Credentialing Committee.
