Scott & White Health Plan (SWHP) Complex Care Guidance

The mission of the SWHP Complex Care Guidance Program is to advocate for members in the assessment, planning, and implementation of care guidance programs in support of the clinical treatment plan and to promote quality and cost-effective interventions and outcomes through advocacy, communication, and resource management.

The SWHP Complex Care Guidance program includes a team of registered nurse case managers who advocate for patients and strive to assist them to achieve the greatest value possible from their health care plan. Case Managers facilitate arrangements to meet care needs for patients with chronic illnesses or catastrophic illnesses or injuries. Our program is holistic, patient-centric, and facilitates the transition of the patient to the most appropriate care setting and care guidance programs along the continuum of care.

Case managers work with the physician, member, and family to set goals and develop a plan for self-management to assist them in reaching their previous or optimum health status.  

Complex Care Guidance endeavors to assist the physician by supporting his/her plan of care, providing assistance in the coordination of services, and providing timely and meaningful information.

Participation is voluntary and is available at no additional cost to Members.
