IMPORTANT INFORMATION REGARDING CLAIMS SUBMISSION

Effective immediately, any paper claims received by Scott & White Health Plan that do not have the correct 11-digit member number on the claim, including the correct member suffix, will be rejected.
The member number is located on the member's ID card.
