Adding Providers to Existing Agreements
In order for new providers to receive payment under their Participating Group Agreement during the credentialing process, the following must be adhered to:
- Notify SWHP Provider Relations of new employee (new Group Provider) using SWHP Provider Data Sheet (PDS) - found at www.swhp.org explaining the reason for the request
- Upon receipt of PDS, SWHP Provider Relations will create a legal Certification to add new provider to the existing Group Agreement and forward for signature
- Group Provider will sign Certification and submit it along with a fully completed Texas Standardized Credentialing Application with appropriate attachments (credentialing checklist found at www.swhp.org providers tab (how to become a SWHP provider)
- SWHP Provider Relations will ensure that basic Credentialing guidelines are met (appropriate licensure, board certification, network admitting privileges & all documents are current) and issue a provisional effective date and communicate date to Group Provider (at this time, the Group provider may directly bill SWHP)
- SWHP Provider Relations will send completed Application for processing
- Group Provider acknowledges that the provisional effective date does not guarantee permanent network participation as this is contingent upon successful completion of the SWHP Credentialing process
- Group Provider will be notified by SWHP Credentialing once credentialing is completed.
Please note: for providers leaving a Group, a new agreement will be required.
