SWHP VitalCare Program Expanding
Scott & White Health Plan is pleased to announce the expansion of the SWHP VitalCare program(s) for our Commercial Group Members!
Managing your health can be overwhelming, but you don't have to do it alone. SWHP is here to help you take care of your health so you can keep your life and family needs in balance. Our goal is to help you feel better and more in control of your own well-being. You are important to us and that's why we're inviting you to participate in the SWHP VitalCare program(s) now available to you and/or your family. Many of you are in the process of being contacted regarding opportunities for participation in the SWHP VitalCare program(s).
The SWHP VitalCare program provides you with the tools and resources to learn more about your health and to continue to make positive changes in your life. You can call and speak to a care coach - a licensed medical professional - to answer questions and help you to understand health conditions you may have. Your care coach will also provide support and encouragement to help you get the most out of your SWHP benefits. You will also receive health brochures, bulletins, notices and other information through mail or phone to support you as you follow your physician's plan of care.
As of early October, the number of programs that you may receive support from have dramatically increased. The condition-specific areas that we are now offering support and care coaching for are:

- Chronic Back Pain
- Chronic Kidney Disease
- Chronic Obstructive Lung Disease (COPD)
- Cognitive Impairment
- Congestive Heart Failure
- Coronary Artery Disease (CAD)
- Crohn's Disease
- Depression

- HIV/AIDS
- Hypertension
- Migraines
- Multiple Sclerosis (MS)
- Obesity
- Osteoarthritis
- Parkinson's Disease
- Post Traumatic Stress
- Rheumatoid Arthritis
- Ulcerative Colitis
