Transition of Care
For enrollees new to SWHP who are in active treatment for medical conditions with non-SWHP network providers, SWHP may grant up to 90 days to transition care from non-SWHP providers to SWHP providers, based on individual case review.
Please fill out the Transition of Care (pdf, 91kb) form and either submit it to SWHP at:
